Before scheduling a first appointment, we will schedule a free 15 minute phone consultation to decide if we are a good fit with one another for therapy. To schedule a phone consultation send me an email with times that you are available for a phone call. It is also helpful to include information regarding your schedule for appointments, and plan for payment with insurance or private pay.
I accept First Choice, Regence, Premera and Lifewise Insurance as an in network provider. For all other insurance networks I can bill out of network by providing monthly statements for you to submit to your insurance provider.
Please call your insurance provider prior to our first meeting to be informed of the following:
Your yearly deductible
Your copay or co-insurance
It is your responsibility to learn about your particular insurance plan. The number for your insurance company should be listed on the insurance card.
If you need to cancel an appointment, please notify me at least 24 hours in advance to avoid being charged for the time. If you do not attend your appointment without a 24 hours notice you will be charged a “No Show Fee” at your regular rate. If you call and cancel with less than 24 hours notice, you will be charged a “Late Cancelation Fee” at your regular rate.
